The season premiere takes place a month after the events of the season 3 finale.
Edie survives her suicide attempt. The neighborhood is shocked to learn of her suicide attempt.
A new family moves into Mike's old house; a married couple Katherine Mayfair and Adam Luther, and 17-year-old Dylan Mayfair, Katherine's daughter and Adam's stepdaughter. The housewives get together to watch them move in. Katherine is a perfectionist like Bree, Adam is a gynecologist who's significantly younger than Katherine, and Dylan is a goody two-shoes who Katherine overprotects, but Dylan really is as boy crazy as Danielle. Katherine and Dylan lived in that same house 12 years ago with Katherine's aunt, Lillian Simms. Lillian is now dying and Katherine, Adam, and Dylan have returned from Chicago, Illinois to take care of her. But that's not the true reason they left Chicago; something dark happened in Adam's business causing them to leave. Every member in this family has a dark secret. Dylan is unhappy with her new bedroom because she can't fit all her belongings in there. She asks about another bedroom that's locked, but Katherine says no and won't let anyone near it. Adam is shocked about the outburst Katherine has about the locked room. After the fight between Katherine and Dylan, Dylan plays her violin intensely, which she does to supress her emotions, ending up with a stream of blood running down her wrist. Dylan was good friends with Julie Mayer when she lived on Wisteria Lane before. Julie shows Dylan around the neighborhood as a refresher but Dylan doesn't remember anything. She doesn't remember the time she got injured in a bike accident, which Julie tells her about. Dylan claims that she didn't learn how to ride a bike until she moved to Chicago. Dylan has suffered from some memory loss, causing the other residents to be suspicious that something traumatic happened in her childhood. In a flashback, 5-year-olds Julie and Dylan are sitting in Susan's house talking about what they want to be when they grow up. Katherine is an old friend of Susan's, but she doesn't get along with the other housewives.
Susan and Mike are trying to settle down into a married life, but that's easier said than done. Susan is worried that Mike's not as happy as he should be. Mike tries to be a father figure for Julie, who's having a hard time getting used to having Mike as a stepdad. Susan goes to the gynecologists' office and is mortified that her regular gynecologist is out and Adam will be her substitute. Susan is very uncomfortable with her new neighbor's face buried in her vagina. Susan finds out she's pregnant.
Lynette starts losing hair and energy because of the chemo. She uses a wig to mask her hair issues but getting past the fatigue is harder. Parker is playing Friar Truck in the school production of Robin Hood. Every year after the play, they have a pancake supper as a reward for the children and the moms take turns hosting it, and this year Lynette's supposed to do it, but she can't because of the cancer. Since she doesn't want anyone to find out of her condition, she comes up with another excuse, but the moms don't believe her so she tells the truth. Lynette's issues with Stella are continuing.
While Bree is shopping for baby shoes, an elderly woman approaches her asking to touch her "pregnant" belly. Bree says no, claiming to be sensitive, but the real reason is she doesn't want anyone to find out she isn't really pregnant. Bree gets physical with the woman when she tries to touch her belly anyway. We find out why Bree is faking a pregnancy.
While shoe shopping with Vern, Gabrielle tells him that she's worried about money and doesn't think she can get a decent job. She is continuing her affair with Carlos. She is desperately trying to keep it a secret from Victor. Victor almost catches Carlos and Gabrielle together.
